Medical Care
Cherry Street Health Services provides Health Care to Grand Rapids area and surrounding communities. The Medical department staff includes Family Practice Physicians, Pediatricians, Internal Medicine Physicians, Nurse Practitioners, Physician Assistants, and Nurse Midwives. The medical team approaches medical problems in a way that addresses the physical, emotional and social issues that can impact wellness. A twenty-four hour nurse triage system provides round the clock contact with patients who need our help no matter what time of day. We have 13 different centers, each providing care for well visits, sick visits as well as ongoing medical care by appointment. Services such as for laboratory, prenatal care, radiology services for general x-ray, mammography and ultrasound are also available.
Dental Care
CSHS offers primary dental services to people of all ages in eight locations. In addition to the various site locations, the CSHS dental program offers care in the public school system, traveling to over 50 inner city schools including Grand Rapids, Kentwood and Wyoming Public School Systems. Special services for emergency/urgent care are provided at West Side Health Center, Cherry Street Health Center, Ferguson Dental Center and Belknap Commons Health Center. We provide full dental services as any general dentist office would provide such as exams, cleanings, x-rays, fillings, crowns, root canals, complete dentures, partials dentures, periodontal treatment along with oral surgery procedures. Our emergency walk-in services are for patients in pain with or with out swelling.
Mental, Psychiatric & Behavioral Health Care
Life can be stressful enough. Getting healthy or staying healthy isn't just for the body. Cherry Street Health Services recognizes that your mind and emotions also need maintenance. That's why we offer licensed therapists who provide individual and family counseling for our patients. The new Durham Clinic at Heart of the City Health Center addresses both mental and physical concerns in one place (616.965.8283), and we offer counseling services at most of our locations. Vision Care
The Grand Rapids Lions Club Vision Clinic at Heart of the City Health Center (formerly our Cherry Street Health Center Vision Clinic), provides the area’s largest eye care program for the uninsured. In October 2011, the clinic moved and expanded services. This could not have happened without the generosity of the Grand Rapids Lions Club and our community partners. Services include dilated eye exams, diagnosis and treatment of ailments, chronic eye problems, refractions and dispensing of eye glasses, as well as case management services for those with diabetes. Eligibility for sliding fee services is based on income and family size. Minimum payment of $15.00 is expected at the time services are provided. Local ophthalmologists and optometrists provide volunteer staffing at the clinic. The eye care program has been in operation since 1988, originated through the efforts of Dr. Mark Sheldon with Grand Rapids Ophthalmology. Equipment and services have been furnished through Grand Rapids Ophthalmology, American Foundation of Vision Awareness, the Grand Rapids Lions Club and the Association for the Blind and Visually Impaired. Maternal Support Services Program
In conjunction with medical prenatal services, the Maternal Support Services program provides counseling, case management, education, transportation, and translation for pregnant women. A registered nurse provides house calls helping women with special needs during pregnancy. A registered Dietitian monitors growth and development during pregnancy. A social worker assists with counseling as well as case management for special needs. These caring professionals carry out their work for pregnant women and the delivery of healthy babies.
WIC- Women, Infants and Children Program
Women's Infant and Children is a state funded nutrition education and food coupon program for pregnant women and children up to age five. A registered Dietitian provides counseling and education as well as healthy food Coupons to encourage proper nutrition during pregnancy and the formative years of childhood. Over 900 patients are served annually, components of the program include monitoring growth and development, screening for lead poisoning, anemia, growth problems, and nutritional needs.
The Health Disparities Collaboratives
The chronic model of care is an approach being used by our clinical staff that converts the traditional medical model approach to one that includes the patient in the implementation of care. The model incorporates the patient as the central focus for self management. As clinical providers, we can impact a patients health care through our limited time with individuals. The medical decisions we make are very important but equally important is an individual's willingness to learn about their own health care and to make positive choices. We are finding success in clinical outcomes because of the motivation that a patient has in their own willingness to participate and get involved in their care. Under the new model, in complement with medical interventions, patients develop goals that are achievable and produce positive outcomes. Pharmacy
Cherry Street opened our new Pharmacy at Heart of the City Health Center in February 2012. Call today to learn more and transfer your prescriptions to our downtown location at 616.965.8278! In addition, our 340B Program provides medications for Cherry Street Health Services Patients by offering discounts through a special federal program for those who are eligible. These special discounts can have savings up to 50% of retail or more. Patients can take their prescriptions to one of these contracted pharmacies and the pharmacy does the rest. Childhood Asthma & Obesity Programs
Heart of the City Pediatrics (formerly Grand Rapids Pediatrics), has implemented a program for children who have asthma and for their families. The program provides an asthma educator to address the needs of parents and the children regarding medication use, inhaler use, exercise and what to do during an attack. Weight management is also offered for children and their families through a 12 week program called STEAM (Set Goals, Take Control, Eat Healthy, Accountability). This clinically proven program promotes activity, education and weight management techniques through clnical expertise of pediatricians and exercise facilitator. Heart of the City Pediatrics also provides case management services for special needs children. Families with children who have chronic illness or those with birth issues have access to pediatricians that specialize in their care. The program provides special assistance with education, referral, resources and case management.
AmeriCorps/Community HealthCorps
The Community HealthCorps team at Cherry Street Health Services is composed of a diverse group of individuals. People of all social and economic backgrounds choose to join our team for a variety of reasons. US citizens or US nationals over the age of 18 with at least a GED or high school diploma are eligible to apply for the program.
Regardless of their background, members become part of a community's struggle for better health and economic independence. Service with the Community HealthCorps team is a valuable experience for anyone with a passion for service and a commitment to improving health care for the underserved. Members receive a living allowance, child care (if eligible), individual health insurance, student loan forbearance and an education award of $5,550 upon successfully completing a term of service. The term of service is a minimum completion of 1,700 hours within a 12 month time frame at full-time status, which is 35 hours or more per week.
Community HealthCorps members also receive training from the Prescriptions for Success, a comprehensive curriculum that covers topics related to serving in a diverse health care environment. Such topics include cultural competency, the primary care environment, patient relations, and case management, to name a few. Extensive training centered on the service environment, professional development, and member requests are also provided.
